The right pelvic, or iliac, veins carry blood from the right lower extremity and pelvic organs into the inferior vena cava, formed by the confluence of the internal and external iliac veins into the common iliac vein. Fluoroscopic venography of this segment is performed with contrast injected from a femoral access point and followed proximally, most often to evaluate thrombus extension from the leg into the pelvis, to assess for extrinsic compression from pelvic masses or lymphadenopathy, or to plan iliac vein stenting. This segment is anatomically important because the right common iliac vein passes beneath the right common iliac artery in a location less prone to the compression seen on the left, so findings here are interpreted with that comparatively lower baseline risk in mind. The study is coded to the right side specifically to distinguish it from left-sided or bilateral pelvic venous imaging.
