Procedure Overview

Fragmentation refers to breaking up a stone or other solid mass inside the urinary tract into smaller pieces without removing the pieces during the same maneuver. Shockwave lithotripsy (ESWL) delivered from outside the body and laser or ultrasonic lithotripsy performed through a ureteroscope are the two most common ways this is done for kidney and ureteral stones, and occasionally bladder stones. The goal is to reduce a stone too large to pass into fragments small enough to be passed naturally or removed with a separate instrument.

The right kidney pelvis is the funnel-shaped collecting chamber where the major calyces converge before urine passes into the ureter, and it is a frequent site for staghorn or pelvic calculi that form when urine stagnates or crystallizes around an obstruction. Fragmentation here typically uses lithotripsy energy, delivered via a percutaneous nephroscope or ureteroscope, to shatter a stone into pieces small enough to pass spontaneously or be removed with a separate extraction procedure. Because the pelvis narrows into the ureteropelvic junction, larger fragments risk becoming lodged there, so the coding distinction matters when documentation separates stone-breaking from any subsequent retrieval. Access route and imaging guidance are usually noted alongside the procedure.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
