Procedure Overview
Fragmentation of the heart and great vessels covers procedures that break up solid material lodged inside a cardiac chamber or blood vessel without removing the fragments from the body. Instead of pulling the obstruction out, the physician uses mechanical force, ultrasonic energy, or a similar technique to shatter it in place, allowing the smaller pieces to be absorbed, dissolved, or carried away naturally by the bloodstream.
This approach is used when a clot or piece of calcified plaque is difficult to grasp and extract whole, or when the vessel's location makes an extraction device impractical. Intracoronary or intravascular lithotripsy for heavily calcified plaque is a modern example, as are certain catheter-based techniques for breaking apart older, organized thrombus that resists suction removal.
The goal is the same as extraction-based procedures - restoring blood flow and reducing the danger of a large obstructing mass - but the method leaves the debris behind in smaller, less obstructive pieces rather than taking it out through the catheter or incision.

The right pulmonary artery carries deoxygenated blood specifically to the right lung, and fragmentation of this vessel addresses calcified or organized thrombotic material, most often related to chronic thromboembolic pulmonary hypertension, that is too rigid for balloon angioplasty alone. Breaking up the obstructing material in place, typically with specialized catheter-based devices, helps restore luminal patency and improve perfusion to the right lung without removing tissue from the body. Because the right pulmonary artery branches extensively and follows a more horizontal course beneath the aorta, catheter manipulation requires attention to distal branch anatomy to avoid embolizing fragments into smaller, harder-to-treat segments. Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope.
