Procedure Overview
Fragmentation breaks solid material within the airway or lung into smaller pieces without removing it in the same step, typically used on a calcified broncholith, an impacted foreign body, or thick, hardened secretions that are too large or firm to extract intact. Mechanical, laser, or other energy-based tools are used to shatter or break up the material so it can pass naturally or be removed afterward through a separate action.

The left upper lobe bronchus divides into segments supplying the anterior, apicoposterior, and lingular portions of the upper lobe, and its relatively narrow branching pattern makes it susceptible to plugging by thickened secretions, blood clots, or aspirated material. Fragmentation is performed to disrupt this obstructing content into smaller pieces using bronchoscopic mechanical, cryogenic, or laser techniques, without physically removing the fragments during the procedure. Because this bronchus sits close to the pulmonary artery and can be difficult to access endoscopically given its takeoff angle, careful technique is needed to avoid perforation. Documentation should identify the exact segmental bronchus involved and distinguish the fragmentation step from any later extraction of the loosened debris, which is captured under a separate root operation.
Approach: Percutaneous
