Axillary Vein, Right

The right axillary vein continues from the brachial veins in the upper arm and becomes the subclavian vein as it crosses the outer border of the first rib, making it accessible for device insertion via an infraclavicular approach used in pacemaker and defibrillator lead placement as an alternative to direct subclavian puncture. Insertion of a catheter or lead into this vessel is often favored because axillary vein access, particularly when done under ultrasound guidance, carries a lower risk of pneumothorax and avoids the costoclavicular compression associated with a more medial subclavian stick. Its position lateral to the first rib, outside the tight space between the clavicle and rib, is the key anatomic reason this site is chosen over the subclavian vein for long-term implanted devices.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
