Procedure Overview

These procedures are performed when a patient cannot maintain their own airway, needs mechanical ventilation, or requires ongoing respiratory monitoring, ranging from emergency intubation to planned tracheostomy for chronic ventilator dependence.

Lung, Left

The left lung's two lobes and longer, more horizontal main bronchus make it anatomically distinct from the right lung when a device is inserted into its tissue. Insertion here usually involves fiducials or fixation markers placed to guide radiation targeting of a tumor, or occasionally a monitoring device left in the parenchyma after a procedure. The left lung's proximity to the heart and aorta requires careful image guidance during percutaneous placement to avoid vascular injury, and the mediastinal shift caused by the heart can affect needle trajectory planning. As with the right lung, coders should confirm the device is seated within lung tissue itself, since a catheter left in the adjacent pleural space would instead be coded to the pleura.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Device: Infusion Device

Infusion Device denotes a device left in place to deliver medication, fluids, or other substances into a body part over time, such as an intrathecal or epidural pump. It is distinct from devices that merely monitor or mechanically support tissue, since its function is ongoing pharmacologic or fluid delivery rather than structural replacement. Common placements include the spinal canal, peritoneal cavity, and vascular access sites.

Coders assign from this family when documentation describes placing a device without disturbing or excising native anatomy, and the device stays behind after the procedure ends. Operative notes should specify the device type (endotracheal tube, tracheostomy tube, monitoring device) and the body part entered, since that determines the qualifier and body part value. A frequent error is coding a tracheostomy tube exchange or routine reinsertion as Insertion when it should reflect the original placement versus a change, or confusing airway securement during anesthesia (not separately coded) with a therapeutic tracheostomy tube placement. Coders also sometimes miss that a percutaneous versus open tracheostomy approach changes the approach character even though the root operation stays the same.
