Inspection procedures of the eye involve a surgeon visually or manually examining the eyeball, eyelid, conjunctiva, or surrounding structures without removing tissue or fixing a problem during that same act. The purpose is diagnostic: confirming the extent of an injury, checking the position of an implant, evaluating unexplained vision changes, or looking for foreign material after trauma. These procedures may be done with an operating microscope, an endoscope threaded through a small incision, or by direct manual palpation depending on what part of the eye needs to be examined.

A patient might undergo an eye inspection after blunt trauma to rule out a ruptured globe, before deciding whether a more extensive repair is warranted, or as part of working up a sudden change in vision when imaging alone hasn't given a clear answer. Because the goal is purely exploratory, inspection is often a bridge to another procedure performed in the same operative episode rather than a standalone treatment.

Recovery expectations depend heavily on how invasive the approach was. A brief external inspection under topical anesthesia carries almost no downtime, while an inspection requiring an incision into the orbit involves the same precautions as any intraocular surgery, including protecting the eye from pressure and infection during initial healing.

Inspection of the left extraocular muscle is the direct examination of one of the muscles governing eye movement, performed to investigate abnormal ocular alignment, suspected muscle slippage, or the aftermath of orbital trauma or prior strabismus surgery. Reaching the muscle requires incising the conjunctiva and Tenon's capsule to expose its insertion on the sclera, allowing the surgeon to assess whether the muscle is intact, properly tensioned, and correctly positioned. This step is frequently diagnostic, clarifying the cause of double vision or limited gaze before any corrective maneuver is planned. It is coded on its own only when no repositioning, repair, or other definitive action is taken on the muscle during the same operative encounter, since those would be reported under their respective root operations.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Coders assign Inspection only when the operative note describes examination as the entire objective of that portion of the procedure and no tissue is taken, altered, or repaired. Documentation needs to specify the approach used - external, percutaneous, or via natural or artificial opening - since that detail changes the code, not just the fact that the surgeon "looked at" the eye.

The most common mistake is coding Inspection separately when it was actually a routine, inherent step of a more definitive procedure performed through the same incision. ICD-10-PCS guidelines direct coders to capture only the more extensive procedure in that scenario; the inspection component is not coded again unless the surgeon documents examining a different body part with no other procedure performed on it. Coders also need to confirm the specific eye structure examined, since "eye" alone is too vague when the note distinguishes the orbit, conjunctiva, or globe.

ExtractionInspection is frequently confused with Extraction or Excision when a biopsy is taken during the same visit - if tissue is removed for pathology, the root operation shifts to reflect that removal rather than mere inspection.
RepairIt is also distinguished from Repair, since Inspection never involves restoring a body part; if the surgeon fixes what they find, code the corrective action instead.
