Procedure Overview

Inspection procedures on the upper joints involve a surgeon looking at, and often feeling, a joint such as the shoulder, elbow, wrist, or the small joints of the hand and fingers to determine its condition, without any tissue being cut out, repaired, or replaced. This is done either through a small camera inserted through a tiny incision (arthroscopy) or, less commonly, by opening the joint directly for a visual and manual exam. It is typically ordered when a patient has unexplained joint pain, swelling, instability, or a mechanical symptom like catching or locking, and imaging alone has not given a clear answer.

Because the goal is purely diagnostic exploration, an inspection is often the first step in a same-session procedure - the surgeon looks around, decides what is wrong, and then may move directly into a repair or other corrective step within the same operative episode.

Thoracic Vertebral Disc

The thoracic vertebral discs lie between the twelve thoracic vertebral bodies and are inherently more constrained in their bulk and mobility than cervical or lumbar discs due to the stabilizing effect of the rib cage and sternum. Inspection of a thoracic disc is far less common than at other spinal levels but is performed when imaging identifies a herniation or degenerative change believed responsible for myelopathy or radicular symptoms at a specific thoracic level, since thoracic disc herniations carry a heightened risk of spinal cord compression given the narrow canal diameter in this region. Direct inspection, whether through open thoracotomy, video-assisted thoracoscopic approach, or posterior access, is typically performed immediately before planned discectomy to confirm the level and the disc's precise relationship to the adjacent cord.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Coding & Documentation

A code from this family is appropriate only when inspection is the sole objective of the encounter, or when it is performed on a body part separate from any other procedure done during the same operative episode. Documentation should describe the joint examined, the approach (percutaneous endoscopic for arthroscopy, or open), and confirm that no biopsy, repair, or other definitive intervention was carried out on that same joint.

The most common assignment error is coding an inspection separately when it was actually the exploratory portion of a more definitive procedure on the same joint - PCS guidelines direct that the inspection is not coded separately in that case, since it is inherent to gaining surgical access. Coders also sometimes miss that diagnostic arthroscopy converted to a therapeutic one changes the correct root operation entirely.

Commonly Confused With

ExcisionInspection is frequently confused with diagnostic biopsy procedures classified as Excision with a diagnostic qualifier, since both may be done arthroscopically to evaluate joint disease - the distinguishing factor is whether tissue was actually removed for pathologic study.
ReleaseIt is also confused with Release or Repair when the operative note describes lysis of adhesions or debridement performed during the same look, in which case the more definitive root operation, not Inspection, is coded for that joint.
