Procedure Overview

Bronchoscopy is the most common example, where a flexible scope is passed through the airway to visually assess the lining of the trachea and bronchi, check for tumors, bleeding, or foreign bodies, or evaluate an unexplained cough or abnormal imaging finding. Mediastinoscopy and thoracoscopy performed purely to look inside the chest also fall here when no additional therapeutic action is taken during the same encounter.

Tracheobronchial Tree
The tracheobronchial tree encompasses the trachea and the full branching network of bronchi down to the segmental level, and inspecting it as a whole typically means a diagnostic bronchoscopy that surveys the airway from the vocal cords through both lung bronchial systems rather than a single named segment. This global inspection is used to evaluate diffuse processes such as widespread mucosal abnormalities, multifocal endobronchial lesions, diffuse hemoptysis of uncertain origin, or to assess airway patency after transplant or prolonged intubation. Because the exam covers multiple named branches, it is coded to the tracheobronchial tree rather than to an individual bronchus, and any biopsy or intervention performed at a specific site during the same exam is coded separately using the corresponding specific body part.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Inspection is frequently confused with Excision or Extirpation performed via bronchoscopy, such as when a biopsy or foreign body removal happens during the same scope pass; in those cases the therapeutic root operation is coded instead of, not in addition to, Inspection. It is also distinct from diagnostic imaging procedures, which use radiologic technology rather than direct visualization or palpation to evaluate the airway.
