A patient might encounter it during an epidural injection, a course of intravenous antibiotics, contrast dye given before a CT scan, or tube feeding after surgery; each represents a substance being placed into the body to achieve a specific therapeutic or diagnostic goal rather than a physical alteration of tissue.

Pericardial Cavity

The pericardial cavity, the thin fluid-filled space between the heart and its fibrous sac, is accessed for substance introduction most often in the setting of malignant pericardial effusion, where a sclerosing agent is instilled after drainage to prevent fluid from reaccumulating. Chemotherapeutic agents may also be introduced locally to treat cancer that has spread to the pericardium. Access is typically obtained through a pericardial catheter left in place following pericardiocentesis or a pericardial window procedure, allowing the substance to be instilled once the cavity has been adequately drained. Because the pericardial space lies immediately adjacent to the heart, careful catheter position and slow administration reduce the risk of arrhythmia or hemodynamic compromise, and documentation should confirm catheter placement was verified before the substance was introduced.

Approach: Percutaneous

In Administration, Percutaneous denotes a substance delivered by needle injection through the skin, such as an intramuscular or subcutaneous injection, without cutting or scope guidance. It differs from External, where the substance is applied to the skin surface itself, and from Via Natural or Artificial Opening, where it is introduced through an orifice rather than through a puncture.
