Procedure Overview

This family covers magnetic resonance imaging of the bones of the arms and hands - the scapula, clavicle, humerus, radius, ulna, wrist bones, and fingers, along with the joints and soft tissue that surround them. Instead of using radiation, the scanner uses a strong magnet and radio waves to excite hydrogen atoms in the tissue, and a computer translates the signal into detailed cross-sectional and multiplanar pictures. Because it shows cartilage, ligaments, tendons, marrow, and muscle far better than an x-ray, it is often ordered after plain films come back normal but pain or swelling persists.

A clinician typically orders this study to look for occult fractures that don't show up on x-ray, to evaluate cartilage or ligament tears around the shoulder, elbow, or wrist, to assess bone marrow for infection or tumor, or to plan surgery on a complex fracture. It is also used to follow up known lesions such as bone cysts or avascular necrosis of the humeral head. Patients with pacemakers or certain metal implants may need an alternative study, and the exam can take thirty minutes or more with the limb held still inside the scanner.

Upper Arm, Right

MRI of the right upper arm evaluates the soft tissue compartment surrounding the humeral shaft, including the biceps and triceps muscles, neurovascular structures, and any soft tissue masses, making it distinct from imaging focused on the adjacent shoulder or elbow joints. It is most often used to characterize soft tissue tumors, muscle tears, or hematomas in this segment, and to assess the extent of infection or abscess when clinical findings suggest deep tissue involvement. The upper arm's relatively large muscle bulk and straightforward anatomy compared to the joints above and below it allow for good soft tissue contrast, but the field of view must be positioned carefully to capture the full length of the segment relevant to the clinical question. This body part designation applies specifically to the arm shaft between the shoulder and elbow, not to either adjacent joint.

Contrast: Other Contrast

Other Contrast captures imaging studies using a contrast medium that is neither high- nor low-osmolar iodinated agent, such as barium sulfate for gastrointestinal studies or gadolinium-based agents for certain applications outside standard MRI classification within this axis. It is used when the contrast administered does not fit the osmolarity-based iodinated categories, distinguishing it from those specific classes and from studies performed with no contrast.

Qualifier: Unenhanced and Enhanced

Unenhanced and Enhanced specifies that an imaging study was performed both before and after administration of contrast material, allowing comparison between the two phases. This is common in studies evaluating enhancement patterns of lesions, such as certain CT or MRI protocols. It differs from None, which reflects a purely unenhanced study, and from single-agent contrast qualifiers used elsewhere.

The coder selects the specific body part value based on which bone or joint region the radiologist actually imaged and reported on - shoulder, elbow, wrist, and hand each have distinct values, and coding the wrong segment because the order form says "upper extremity" without specifying is a common error. Documentation should state whether contrast was used, since that changes the qualifier, and whether the study was unilateral or bilateral if the body part choice allows for that distinction. The report needs to clearly identify laterality (right versus left); relying on the order alone rather than the radiologist's findings can lead to a mismatch. Another frequent mistake is defaulting to a CT or x-ray code out of habit when the modality section of the report clearly documents MRI.

Commonly Confused With

It is easily confused with the CT scan and plain radiography families for the same body region, since all three can be ordered to evaluate the same fracture or joint complaint. The distinguishing factor is the imaging technology itself: MRI uses magnetic fields and radiofrequency signals with no ionizing radiation, CT uses multiple x-ray exposures reconstructed by computer, and plain radiography captures a single static image on a photographic plate. It is also sometimes confused with ultrasonography of the same region, but ultrasound is a real-time, sound-wave-based study usually reserved for soft tissue or tendon assessment rather than bone marrow or occult fracture evaluation.
