The right lower lobe bronchus is the airway feeding the largest lobe of the right lung, branching into superior and basal segmental divisions shortly after its origin from the bronchus intermedius. Occluding it is most often performed for massive or recurrent hemoptysis originating in that lobe, for a persistent air leak or bronchopleural fistula the lobe cannot be resected to treat, or to protect the rest of the lung during endobronchial valve placement for severe emphysema. Because the lobe divides quickly into several segmental branches, the occluding device or material must sit proximal enough in the lobar airway to seal all downstream segments, and bronchoscopic access is complicated by its take-off angle just past the middle lobe orifice.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
