Main Bronchus, Right

The right main bronchus, shorter and more vertically angled than its left counterpart, is occluded to completely block airflow through it, commonly to manage a persistent bronchopleural fistula, uncontrolled air leak after lung surgery, or massive hemoptysis when the bleeding source cannot be otherwise controlled. This is typically achieved bronchoscopically with an endobronchial blocker, valve, or sealant advanced through the airway and positioned at the origin of the right main bronchus. Because this bronchus supplies the entire right lung, occlusion here effectively excludes the whole lung from ventilation, so it is used selectively and often as a bridge to definitive surgical management, with the patient's tolerance of single-lung ventilation considered beforehand.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
