Procedure Overview

This family covers plain film x-ray imaging of the urinary tract, including the kidneys, ureters, and bladder, sometimes performed together as a single "KUB" study. A conventional x-ray beam passes through the abdomen and is captured on a digital or photographic plate, producing a single flat image that shows the size, shape, and position of urinary structures along with any calcified stones or foreign objects. It is one of the fastest and least expensive ways to get an initial look at the urinary system.

The study is typically ordered when a patient has flank or abdominal pain suggestive of a kidney stone, to check the position of a ureteral stent or catheter, to look for radiopaque foreign bodies, or as a follow-up to see whether a previously identified stone has moved or passed. Because a plain film cannot show soft tissue detail the way CT or ultrasound can, it is often a first step rather than the final word, with more advanced imaging ordered afterward if the plain film findings are inconclusive or if bowel gas obscures the structures being evaluated.

Ureter, Left

The left ureter runs from the renal pelvis down along the psoas muscle, crosses the pelvic brim near the iliac vessels, and enters the bladder from the left side, and plain radiography of this course serves largely the same narrow purpose as on the right: localizing radiopaque stones at the three sites where the lumen naturally narrows, or confirming the position of a ureteral stent or surgical clip. The descending colon and its gas pattern can overlie the left ureteral course, which sometimes complicates interpretation more than on the right side. Since the ureter has no inherent radiographic contrast of its own, findings on this view are essentially limited to calcified stones, indwelling hardware, or wall calcifications rather than any assessment of the ureteral wall or lumen itself.

Contrast: Low Osmolar

Low Osmolar denotes use of a low-osmolar iodinated contrast agent during an imaging study, the class most commonly used in modern radiographic and CT imaging because it better approximates the osmolality of blood and carries a lower risk of reaction than High Osmolar media. This value distinguishes studies performed with this now-standard contrast type from those using older high-osmolar agents or no contrast at all.

Coding & Documentation

The coder must confirm from the order and report exactly which structures were imaged - the kidney alone, the ureter, the bladder, or the combined KUB view - since each has a distinct body part value and a single film covering the whole urinary tract does not automatically justify coding every component separately. Documentation should also clarify whether the study is a stand-alone plain film or one image obtained as part of a fluoroscopic procedure such as an IVP, because those belong to different root operations even though both use ionizing radiation. A common error is coding a scout or preliminary film taken immediately before a fluoroscopic contrast study as its own separate plain radiography procedure rather than recognizing it as part of the fluoroscopic exam.

Commonly Confused With

The main point of confusion is with fluoroscopy of the urinary system, which also uses x-rays but produces continuous real-time images, often with contrast, rather than a single static exposure - the distinguishing clue is whether the report describes dynamic, contrast-enhanced observation or a single captured frame. It is also distinct from CT of the urinary tract, which reconstructs cross-sectional images from many exposures rather than producing one planar picture, even though both plain film and CT are frequently ordered to evaluate the same suspected kidney stone.
