Plain radiography of the female reproductive system uses a brief burst of external X-rays captured on a plate to create a flat, two-dimensional image of pelvic structures. It has largely been supplanted by ultrasound and MRI for evaluating the uterus and ovaries directly, but a plain film is still used in specific situations, such as locating a retained intrauterine device, assessing pelvic bony anatomy alongside soft tissue, or as part of a hysterosalpingogram sequence.
The test is fast and produces a single static image rather than a moving or layered view, so it works best for questions with a clear anatomic answer - is a device present, has an object migrated, are there calcifications - rather than for characterizing soft-tissue masses. Because it uses ionizing radiation, it is avoided during pregnancy unless clinically necessary.

Vagina
The vagina is a fibromuscular canal extending from the cervix to the vulva, and like other pelvic soft-tissue structures it produces little inherent contrast on a plain film. A radiograph of this region is typically obtained to localize a radiopaque foreign body, such as a retained pessary, vaginal packing with radiopaque marker, or a lost tampon applicator, or to assess bony pelvic relationships in trauma or obstetric evaluation where vaginal position relative to the pelvis matters. Because the vaginal walls themselves are not visualized, the study's clinical value rests on detecting an unexpected opacity or confirming the absence of one, and coding should reflect that this is a scout-type survey rather than a study capable of demonstrating vaginal wall thickness, masses, or fistulous tracts.
Contrast: High Osmolar
High Osmolar identifies imaging studies performed using a high-osmolar iodinated contrast agent, an older class of media with osmolality well above that of blood plasma. These agents carry a comparatively higher risk of adverse reactions and are used less often today than Low Osmolar agents, which produce similar radiographic enhancement with better patient tolerance. The value simply records which contrast class, if any, was administered for the study.
Coding & Documentation
Coders assign this family based on documentation that a static X-ray image, not a real-time or contrast-enhanced fluoroscopic series, was obtained, and that the field of view included female reproductive structures specifically. A common error is coding a hysterosalpingogram here when the procedure actually involved continuous fluoroscopic contrast imaging, which belongs in the fluoroscopy family instead. The report should also specify whether contrast material was used, since plain radiography by definition does not include contrast administration.
Commonly Confused With
The closest point of confusion is fluoroscopy of the female reproductive system, particularly hysterosalpingography, which looks similar in intent (imaging the uterus and tubes) but uses continuous real-time X-ray with contrast rather than a single static exposure. Ultrasonography and MRI of the same body system are also often ordered for overlapping indications but rely on sound waves or magnetic fields instead of ionizing radiation, and typically offer superior soft-tissue detail.
