Procedure Overview

Plain radiography of the upper arteries captures a static image using external x-ray exposure onto film or a digital detector, producing a flat picture of bone, soft tissue density, and, when contrast is present, the vessel outline. On its own, without contrast, a plain film shows little about the arteries directly, since blood vessels are not naturally distinct from surrounding tissue on x-ray, so this family is most often applied where calcification within an artery wall, a foreign body, or an implanted device such as a stent is visible.

Vertebral Artery, Right

The right vertebral artery arises from the subclavian artery and ascends through the transverse foramina of the cervical vertebrae before entering the skull to help form the posterior circulation of the brain. Radiographic study of this vessel, performed with contrast, is used to evaluate dissection, dolichoectasia, stenosis, or compression from osteophytes and cervical spondylosis, particularly in patients with posterior circulation symptoms like vertigo or ataxia. Because the artery travels through a narrow bony canal for much of its length, imaging can also reveal extrinsic compression related to head rotation or cervical instability. The right vertebral artery is frequently smaller than the left, a normal variant that must be distinguished from pathologic narrowing when interpreting the study. Its course near the craniocervical junction also makes it relevant in trauma evaluation.

Contrast: High Osmolar

High Osmolar identifies imaging studies performed using a high-osmolar iodinated contrast agent, an older class of media with osmolality well above that of blood plasma. These agents carry a comparatively higher risk of adverse reactions and are used less often today than Low Osmolar agents, which produce similar radiographic enhancement with better patient tolerance. The value simply records which contrast class, if any, was administered for the study.

Commonly Confused With

This family is easily confused with fluoroscopy of the upper arteries, which is the modality actually used for most diagnostic arteriograms because it captures contrast moving through the vessel in real time; a plain film cannot show flow. It is also distinct from CT of the upper arteries, which reconstructs cross-sectional and three-dimensional vascular anatomy from many exposures rather than producing a single flat image, and the report's description of a single static exposure versus a series or reconstruction is what separates the two.
