Spermatic Cord, Right

The right spermatic cord bundles the vas deferens, testicular artery, pampiniform venous plexus, cremasteric fibers, and genital branch of the genitofemoral nerve into a single structure running from the inguinal canal to the testis. Reattachment becomes necessary when this cord has been transected, most commonly from penetrating trauma, an inadvertent surgical injury during inguinal hernia repair, or a self-inflicted or assault-related laceration, and the goal is to restore both the tubular conduit for sperm transport and the vascular supply that keeps the testis alive. Repair requires meticulous layer-by-layer reapproximation, often under magnification, to align the vas deferens lumen precisely while separately anastomosing the fine testicular vessels. Because a poorly aligned vas can still leave the testis viable but infertile, the operative note should distinguish vascular from ductal continuity when both were repaired.

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure.
