Procedure Overview

Tendon release frees a tendon from an abnormal physical constraint, such as scar tissue, adhesions, or a tight surrounding sheath, that is restricting its normal movement or causing pain. This is the procedure behind common conditions like trigger finger, where the A1 pulley is divided to let the flexor tendon glide freely again, and de Quervain's tenosynovitis, where a thickened tendon sheath is opened to relieve compression on the tendons underneath.

Release is also used for conditions like tennis elbow when conservative treatment fails, where fibrotic or degenerated tissue tethering the tendon is divided to reduce tension and pain, and for contractures where a tendon has become abnormally short or bound down, limiting joint motion. The tendon tissue itself is not cut through or removed; only the constraining material around it is divided or released.

Ankle tendons on the right, including the Achilles, peroneal, tibialis posterior, and extensor tendons as they pass through the retinacula around the joint, are prone to becoming bound down by scar or thickened fibrous tissue after fracture, repetitive overuse, or prior surgery in this densely packed region. Release cuts this constricting tissue to restore normal gliding and range of motion, and is also used to lengthen a tight Achilles tendon contributing to equinus deformity. Because so many tendons converge around the malleoli in narrow synovial sheaths, the surgeon must isolate the specific tendon causing the restriction while protecting the others and the adjacent tibial or sural nerve branches. Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
