Procedure Overview
Release procedures free a respiratory structure from an abnormal band, scar tissue, or other constraint that is restricting its normal position or movement, without cutting into or removing the structure itself. A typical example is surgically freeing a lung that is trapped by adhesions or a fibrous peel following chronic infection or hemothorax, sometimes called decortication when the entrapping tissue coats the lung surface. Release can also apply to freeing the trachea or a bronchus from surrounding scar tissue or a constricting band that is impairing airflow or causing pain.

Diaphragm
The diaphragm is the dome-shaped musculotendinous partition separating the thoracic and abdominal cavities, and it can become tethered by adhesions, scar tissue from prior thoracic or upper abdominal surgery, or fibrotic bands that restrict its normal excursion during breathing. Freeing these constraints without cutting the diaphragm itself qualifies as a release, since the goal is to restore the muscle's natural range of motion rather than to excise or repair tissue. This is commonly performed in conjunction with lung or esophageal procedures where adhesions have fixed the diaphragm to adjacent structures, impairing ventilation. Because the diaphragm is thin and richly innervated by the phrenic nerve, care in dissection is documented to distinguish adhesiolysis from any incidental repair of the muscle, and the approach (open thoracotomy, laparoscopic, or thoracoscopic) is noted separately from the release itself.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
