Operation Definition

Freeing a body part from an abnormal physical constraint by cutting or by the use of force

Procedure Overview

Release procedures free a body part in the mouth or throat from an abnormal band of scar tissue, adhesion, or other constraint that's restricting its movement or function, without cutting into or removing any of the structure being freed. Typical targets include a tight lingual frenulum limiting tongue movement (tongue-tie), scar bands restricting jaw opening or swallowing after prior surgery or radiation, or adhesions binding the soft palate.

The surgeon cuts or otherwise divides the constraining tissue itself, and by definition, only that surrounding tissue is taken out, not any part of the body part being released. This differs from cutting into the structure itself, and the goal is restoring normal range of motion or function, such as improving a child's ability to nurse or speak, or restoring a patient's ability to open their mouth fully.

Recovery is usually straightforward, with stretching exercises sometimes recommended afterward to prevent the band from reforming.

Epiglottis

The epiglottis is the leaf-shaped cartilaginous flap that folds over the laryngeal inlet during swallowing to prevent aspiration, and scarring here can follow burns, caustic ingestion, radiation, or prior laryngeal surgery, tethering the flap so it no longer moves freely to protect the airway. Restricted epiglottic motion can paradoxically worsen aspiration risk or interfere with normal swallowing coordination, so freeing adhesive bands aims to restore the flap's natural hinge-like excursion rather than reshape or excise the cartilage itself. Because the epiglottis sits at the junction of the oropharynx and larynx, procedures here are usually performed endoscopically, and documentation should distinguish this mobility-restoring release from resection of epiglottic tissue.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Release is assigned when the documentation describes freeing a structure from an abnormal physical constraint - scar tissue, adhesions, or a tight band - by cutting or manipulation, with only the constraining tissue being taken out rather than the target body part itself. The note should identify what was constraining the structure (adhesions, fibrous band, frenulum) and confirm that the body part being freed was not itself excised.

The most frequent coding error is confusing Release with Division, which is used specifically for the spinal or cranial nerves root operations table and not applicable here, or mistaking it for Excision when the surgeon actually cuts into and removes a portion of the constrained structure rather than just the tissue binding it. Another common mistake is failing to identify that a frenectomy or frenotomy freeing tongue-tie is coded as Release rather than Excision, since the objective is freeing movement rather than removing tissue for its own sake.
