This region is frequently affected by fibrosis from chronic reflux disease, prior hiatal hernia repair, or thoracic surgery, and its proximity to the diaphragm, vagus nerve trunks, and the gastroesophageal junction means that release procedures must be performed with attention to preserving the mechanisms that prevent reflux and coordinate swallowing. Because the lower esophagus transitions into the abdominal cavity, surgeons approaching this segment may work through either a thoracic or abdominal route. The procedure is coded here only when the intent is to free the segment from restrictive tissue rather than to divide the muscular fibers of the lower esophageal sphincter itself.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
