Release of the middle esophagus frees this thoracic segment from surrounding scar tissue, adhesions, or fibrotic bands, most often the sequela of prior thoracic surgery, esophagitis, or mediastinal fibrosis, without cutting into the esophageal lumen or removing tissue from the organ. The middle esophagus runs behind the trachea and left mainstem bronchus and anterior to the vertebral column, so constricting tissue in this region can also compress the airway or restrict normal peristaltic motion, contributing to dysphagia distinct from a structural stricture. Because of its proximity to the aorta, azygos vein, and thoracic duct, dissection to mobilize this segment is performed carefully to avoid vascular or lymphatic injury. This root operation is documented separately from esophagectomy or other excisional procedures on the same segment, since the esophageal tissue itself is left intact and only the constraining tissue is divided.

Approach: Via Natural or Artificial Opening Endoscopic

Via Natural or Artificial Opening Endoscopic describes instrumentation introduced through a natural orifice or a surgically created opening while using an endoscope for visualization, as in a routine colonoscopy performed through the anus. It differs from the plain Via Natural or Artificial Opening value by the added scope, and from Percutaneous Endoscopic by using an existing passage rather than a new skin puncture.

A common coding error is assigning Release when adhesions were incidentally taken down simply to gain surgical access, rather than as a deliberate therapeutic step to relieve an obstruction or entrapment - in ICD-10-PCS, incidental lysis of adhesions performed only to reach the operative site is not coded separately.
