Procedure Overview
Release procedures free a female reproductive organ from something abnormal that is constraining it, such as scar tissue, adhesions, or a band of fibrous tissue pressing on the uterus, ovary, or fallopian tube. The surgeon cuts or otherwise divides the constricting material, but importantly does not remove any part of the organ itself - the goal is simply to relieve the abnormal pressure or restriction so the structure can move and function normally again.
This type of surgery is commonly performed for conditions like pelvic adhesions from prior surgery, endometriosis, or infection, which can bind organs together and cause chronic pain, infertility, or bowel and bladder symptoms. Lysis of adhesions around the ovaries and tubes is a frequent example, often done laparoscopically, with the aim of restoring normal anatomy and improving comfort or fertility outcomes.

Hymen
The hymen is the thin membranous fold partially encircling the vaginal orifice, and in some individuals it forms an imperforate or microperforate configuration that obstructs the normal outflow of vaginal secretions or menstrual blood. Release is coded when the surgeon incises or divides this obstructing membrane to open a functional passage, a procedure typically performed in adolescents presenting with cyclic pelvic pain and a bulging membrane from trapped menstrual blood, or hematocolpos. Because the hymen is a thin, largely avascular structure, release here is usually a limited incision rather than an excision of tissue, though documentation should specify whether any hymenal tissue was actually removed, which would instead be coded as excision. This distinction matters clinically since simple incision preserves more of the native anatomy than does hymenectomy.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.

A Release code applies when the documentation specifies that adhesions, scar tissue, or another constricting structure were cut or lysed to free an organ, with the organ itself left intact. Coders should identify which body part was freed - the code is assigned to the organ being released, not to the adhesion tissue that was divided. The most frequent error is confusing simple adhesiolysis with an excisional procedure; if the surgeon actually cuts into or removes a portion of the reproductive organ rather than just the surrounding restrictive tissue, Release is the wrong root operation. Another pitfall is failing to code Release separately when it is performed as a distinct, medically necessary step alongside a different main procedure, versus when it is incidental and not separately reportable.
