Procedure Overview

Release procedures free a lower joint from something that is abnormally restricting its motion, such as scar tissue, adhesions, or a tight, contracted joint capsule. Unlike a repair, which fixes damaged tissue, a release specifically addresses a structure that is constraining the joint from the outside or is fibrosed in place, cutting or manipulating it loose so the joint can move through a fuller range again. Conditions treated this way include arthrofibrosis after a prior surgery, a stiff frozen joint, or adhesions from long-standing inflammation.

Patients are usually referred for this procedure after physical therapy and other conservative measures have failed to restore joint mobility, and the goal is functional improvement in range of motion rather than correction of a structural defect.

The left knee joint shares the same hinge architecture as the right, with femoral condyles articulating against the tibial plateau beneath a patella stabilized by the extensor mechanism and retinacular fibers. Release procedures on this side commonly treat arthrofibrosis after ligament repair, a tight iliotibial band contributing to patellar maltracking, or scar adhesions limiting terminal extension, a particularly disabling loss given the knee's weight-bearing role. Surgeons must specify laterality clearly since bilateral knee pathology is common and left-sided release should never be conflated with the contralateral procedure in the record. As with the right knee, the defining feature is separation of a constraining band or adhesion rather than removal of joint tissue, so coding should reflect a true freeing of motion rather than debridement or partial synovectomy performed in the same setting.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure.
