Freeing a body part from an abnormal physical constraint by cutting or by the use of force
Procedure Overview
Release procedures free a part of the eye that has become abnormally bound down or constrained, most often by scar tissue, adhesions, or fibrous bands that restrict normal movement or function. A common application is releasing adhesions between the eyelid and the eyeball (symblepharon) that can form after chemical burns, severe inflammation, or prior surgery, which otherwise limit eye movement and cause discomfort. Another example is cutting through scar tissue that's tethering the extraocular muscles and limiting how the eye can move.
The surgeon accomplishes this by cutting or otherwise mechanically separating the constraining tissue, without removing the tissue causing the tension or repairing the structure being freed - the objective is solely to eliminate the abnormal restriction. This differs from procedures aimed at correcting the shape or function of the eye part itself.
Patients typically pursue release procedures when scarring has caused pain, restricted eye movement, or cosmetic and functional eyelid problems following an injury, infection, or previous operation. Recovery focuses on preventing the released tissue from re-adhering, sometimes requiring temporary spacers or ongoing eye movement exercises.

The lens in the right eye is the transparent, biconvex structure suspended behind the iris by zonular fibers, and here the operation targets adhesions binding the lens or its capsule to surrounding structures rather than the lens material itself. Posterior or anterior synechiae, fibrous bands from prior inflammation, trauma, or previous intraocular surgery can tether the lens capsule to the iris or vitreous face, restricting pupillary movement or capsular mobility. The surgeon uses fine instruments to lyse these adhesions and restore normal anatomic freedom without removing or altering the lens tissue. This is distinct from cataract extraction, and because lens-related procedures frequently combine multiple steps, documentation should clearly separate any synechialysis or capsular freeing from extraction, insertion of an intraocular lens, or other concurrent lens work.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
Coding & Documentation
The operative note must document that tissue causing an abnormal constraint was cut or freed and that nothing was excised in the process; if any tissue is taken out along with the release, the excised portion may warrant its own additional code. Coders need to identify precisely which eye structure was released - conjunctiva, eyelid, or extraocular muscle - since that changes the body part value even when the technique and intent are identical.
A frequent coding error is applying Release when the surgeon actually removed the offending adhesion or scar tissue entirely rather than simply dividing it to relieve tension, which would instead point toward Excision. Coders should also watch for cases where lysis of adhesions is incidental to a larger procedure performed at the same site, which under PCS guidelines is not coded separately.
