Procedure Overview

Release procedures free a respiratory structure from an abnormal band, scar tissue, or other constraint that is restricting its normal position or movement, without cutting into or removing the structure itself. A typical example is surgically freeing a lung that is trapped by adhesions or a fibrous peel following chronic infection or hemothorax, sometimes called decortication when the entrapping tissue coats the lung surface. Release can also apply to freeing the trachea or a bronchus from surrounding scar tissue or a constricting band that is impairing airflow or causing pain.

Lung, Left

The left lung, with its two lobes and cardiac notch accommodating the heart's leftward position, can become diffusely restricted by pleural adhesions or fibrous encasement following infection, hemothorax, or prior thoracic intervention. Release addresses this by freeing the entire lung from surrounding restrictive tissue so it can expand fully within the pleural space, without removing any lung tissue itself. Given the heart's proximity on this side, dissection near the mediastinal pleura requires particular caution to avoid injury to the pericardium or great vessels. This procedure is coded as Release specifically when the intervention frees the whole lung rather than an isolated lobe or bronchial segment.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
