Procedure Overview
Release procedures free a respiratory structure from an abnormal band, scar tissue, or other constraint that is restricting its normal position or movement, without cutting into or removing the structure itself. A typical example is surgically freeing a lung that is trapped by adhesions or a fibrous peel following chronic infection or hemothorax, sometimes called decortication when the entrapping tissue coats the lung surface. Release can also apply to freeing the trachea or a bronchus from surrounding scar tissue or a constricting band that is impairing airflow or causing pain.

Main Bronchus, Right
The right main bronchus is shorter and more vertically oriented than its left counterpart, branching from the carina to supply the entire right lung, and this anatomic angle makes it more susceptible to compression from adjacent structures such as enlarged mediastinal lymph nodes, vascular anomalies, or postsurgical adhesions. Release of the right main bronchus involves dissecting away this surrounding constrictive tissue to relieve external pressure and restore the bronchus's normal caliber and mobility without removing or altering the bronchial wall itself. Because the right main bronchus lies close to the azygos vein and the right pulmonary artery, careful tissue-plane dissection is needed to avoid injuring these vessels during mobilization. Precise laterality documentation is important to differentiate this procedure from release performed on the left main bronchus or a lobar branch.

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope.
