Freeing a body part from an abnormal physical constraint by cutting or by the use of force

Procedure Overview

Release procedures free ear, nose, or sinus structures that have become trapped or constrained by scar tissue, adhesions, or abnormal bands, without cutting into or removing any of the tissue itself. A typical example is lysis of adhesions between the nasal septum and turbinate that have fused after prior surgery or injury, restoring normal movement and airflow.

Patients undergo this procedure when a structure that should move or function independently has become bound down, often causing obstruction, pain, or restricted motion, such as adhesions limiting eardrum mobility after chronic infection. The surgeon divides only the constraining band while leaving the body part itself intact, distinguishing it from procedures that alter the anatomy of the part being freed.

Release is often needed as a corrective step after previous surgery or scarring rather than as a first-line treatment for a new condition.

Maxillary Sinus, Left

On the left side, the maxillary sinus occupies a similarly large pyramidal space within the maxilla, with its ostium opening into the middle meatus for mucociliary drainage. When scar tissue or adhesive bands form after previous sinus surgery, infection, or polypoid disease, they can pull on the sinus wall or pinch the ostial pathway shut. Release addresses exactly this tethering, cutting or freeing the abnormal fibrous connections so the sinus can drain normally again, without excising any of the sinus's own mucosal lining. Endoscopic instruments allow precise access while protecting the orbital floor above and the dental root apices below, both of which lie close to the maxillary sinus cavity.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

The operative note should make clear that only the constraining tissue was divided, with the target body part left otherwise unaltered.

A common error is confusing Release with Division, since both involve cutting. If the constraining material being cut is itself part of the coded body part, Division applies; if the cutting frees an adjacent structure from that material, Release is correct, and the value representing the site being freed, not the material cut, is what gets coded.
