Procedure Overview
Release procedures free a lower joint from something that is abnormally restricting its motion, such as scar tissue, adhesions, or a tight, contracted joint capsule. Unlike a repair, which fixes damaged tissue, a release specifically addresses a structure that is constraining the joint from the outside or is fibrosed in place, cutting or manipulating it loose so the joint can move through a fuller range again. Conditions treated this way include arthrofibrosis after a prior surgery, a stiff frozen joint, or adhesions from long-standing inflammation.
Patients are usually referred for this procedure after physical therapy and other conservative measures have failed to restore joint mobility, and the goal is functional improvement in range of motion rather than correction of a structural defect.

Metatarsal-Phalangeal Joint, Right
The right metatarsal-phalangeal joints, most notably the first MTP joint at the base of the great toe, bear substantial load during the push-off phase of walking and are a frequent site of capsular tightness from hallux rigidus, bunion deformity, or turf toe injury. Release here frees a contracted joint capsule or adjacent soft tissue, such as the plantar plate or collateral ligaments, that is restricting dorsiflexion needed for a normal gait cycle, and it is often performed alongside but distinct from an osteotomy or exostectomy addressing the underlying bony deformity. Because the first MTP joint differs functionally and structurally from the lesser toe joints, documentation should specify which metatarsal-phalangeal joint was released when multiple toes are involved on the right foot.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope.
