Portal Vein

The portal vein is the main conduit carrying nutrient-rich blood from the splanchnic circulation to the liver, formed by the confluence of the splenic and superior mesenteric veins, and it can become compressed by tumor, pancreatic fibrosis, or adhesions from prior hepatobiliary surgery. Release involves dividing that surrounding constricting tissue to restore portal flow into the liver without cutting or reconstructing the vein itself, which is an important distinction given how frequently portal vein pathology instead requires resection with reconstruction, particularly during pancreaticoduodenectomy. Precise documentation of tissue freed versus vessel manipulated is essential here, since the portal vein's central role in hepatic perfusion means miscoding the extent of intervention has significant clinical and billing implications.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
