Procedure Overview

Release procedures free ear, nose, or sinus structures that have become trapped or constrained by scar tissue, adhesions, or abnormal bands, without cutting into or removing any of the tissue itself. A typical example is lysis of adhesions between the nasal septum and turbinate that have fused after prior surgery or injury, restoring normal movement and airflow.

Patients undergo this procedure when a structure that should move or function independently has become bound down, often causing obstruction, pain, or restricted motion, such as adhesions limiting eardrum mobility after chronic infection. The surgeon divides only the constraining band while leaving the body part itself intact, distinguishing it from procedures that alter the anatomy of the part being freed.

Release is often needed as a corrective step after previous surgery or scarring rather than as a first-line treatment for a new condition.

Sphenoid Sinus, Right

The right sphenoid sinus occupies the body of the sphenoid bone deep in the skull base, immediately adjacent to the optic nerve, internal carotid artery, and pituitary gland, and drains through the sphenoethmoidal recess. Adhesions narrowing this recess, whether from prior surgery or chronic inflammatory disease, can obstruct sinus outflow despite the underlying bone and neurovascular relationships remaining unchanged. Release frees these constricting bands to reopen the natural ostium, distinguishing it from any procedure that would enlarge the opening by removing bone or tissue. Given the sinus's direct proximity to critical neurovascular structures, this is performed transnasally with image guidance and fine endoscopic instruments to keep the dissection confined strictly to scar tissue.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.
