Release of subcutaneous tissue and fascia of the back involves dividing scar tissue or fibrous adhesions across the posterior trunk that restrict normal soft tissue movement, a pattern seen after burns, spinal surgery incisions, or extensive prior debridement. Because the back carries thicker fascia and supports a large surface area, contractures here can limit trunk flexion or the ability to fully mobilize skin flaps needed for wound closure or reconstruction after tumor excision or pressure injury treatment. The procedure targets only the constricting bands, leaving the surrounding tissue intact, which is what separates it from excision. Given the size of the back, the extent of release can vary widely from a small localized scar band to a broad fasciotomy-like division across much of the region.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
