Procedure Overview

Release procedures free a lower joint from something that is abnormally restricting its motion, such as scar tissue, adhesions, or a tight, contracted joint capsule. Unlike a repair, which fixes damaged tissue, a release specifically addresses a structure that is constraining the joint from the outside or is fibrosed in place, cutting or manipulating it loose so the joint can move through a fuller range again. Conditions treated this way include arthrofibrosis after a prior surgery, a stiff frozen joint, or adhesions from long-standing inflammation.

Patients are usually referred for this procedure after physical therapy and other conservative measures have failed to restore joint mobility, and the goal is functional improvement in range of motion rather than correction of a structural defect.

Toe Phalangeal Joint, Right

The toe phalangeal joints of the right foot, the interphalangeal joints between the phalanges of each toe, allow the flexion needed for toe purchase against the ground during gait and are commonly affected by hammertoe or claw toe deformities that produce a fixed or semi-fixed contracture. Release addresses the tightened joint capsule, collateral ligaments, or contracted extensor and flexor tendons tethering the joint in a flexed or extended position, restoring passive motion before or independent of any bony correction such as arthroplasty or fusion performed at the same joint. Because each toe has its own interphalangeal joint or joints, and the great toe has only one while lesser toes have two, the specific digit and joint level released should be clearly documented on the right foot.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
