Freeing a body part from an abnormal physical constraint by cutting or by the use of force
Procedure Overview
Release procedures free a body part in the mouth or throat from an abnormal band of scar tissue, adhesion, or other constraint that's restricting its movement or function, without cutting into or removing any of the structure being freed. Typical targets include a tight lingual frenulum limiting tongue movement (tongue-tie), scar bands restricting jaw opening or swallowing after prior surgery or radiation, or adhesions binding the soft palate.
The surgeon cuts or otherwise divides the constraining tissue itself, and by definition, only that surrounding tissue is taken out, not any part of the body part being released. This differs from cutting into the structure itself, and the goal is restoring normal range of motion or function, such as improving a child's ability to nurse or speak, or restoring a patient's ability to open their mouth fully.
Recovery is usually straightforward, with stretching exercises sometimes recommended afterward to prevent the band from reforming.

Tongue
The tongue is a highly mobile muscular organ essential to speech, swallowing, and airway protection, and its function depends on unrestricted range of motion at the tip, body, and base. Release procedures on the tongue most classically address ankyloglossia, where an abnormally short or thick lingual frenulum tethers the tongue tip to the floor of the mouth, but the same root operation applies to scar contracture from trauma, burns, or prior glossectomy that limits tongue mobility. Because the tongue's intrinsic and extrinsic musculature, lingual nerve, and sublingual vasculature lie close to common release planes, the depth and location of the incision must be carefully controlled to restore mobility without injuring these structures. The procedure is coded as Release when tethering tissue is divided without excising tongue substance.
Approach: External
External approach applies to procedures performed directly on the skin or mucous membrane, or on an accessible body surface, without any instrumentation passing through a puncture or orifice. It covers things like manual reduction of a fracture or excision of a skin lesion. It differs from Via Natural or Artificial Opening in that no internal passage is entered at all, only the exposed surface.
