Carpal Joint, Right

The right carpal joint refers to the articulations among the small carpal bones of the wrist, which may contain implanted hardware such as screws, pins, or small plates used to stabilize fractures or ligamentous injuries involving bones like the scaphoid or lunate. Removal describes extracting such a device once healing is complete, without placing a substitute, and is technically demanding given the small size of the carpal bones and the tight clustering of ligamentous and tendinous structures in this region. Because carpal fixation, particularly for scaphoid nonunion or perilunate injuries, often uses low-profile hardware embedded close to the bone surface, extraction requires precise localization, sometimes with fluoroscopic guidance, to avoid damaging adjacent structures.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Device: Spacer

Coders also sometimes overlook that removal of a device from a joint accompanied by repair of joint damage caused by that device requires an additional Repair or Revision code for the corrected tissue.
