Procedure Overview

Removal procedures in the urinary system take out a device that a clinician previously placed to support, drain, or monitor the kidneys, ureters, bladder, or urethra. Common examples include pulling a ureteral stent that was placed to relieve an obstruction, removing a nephrostomy tube once a kidney has healed enough to drain on its own, or taking out a suprapubic catheter or artificial urinary sphincter component.

These procedures are done when the device has finished its job, has failed, or is causing a complication such as infection or encrustation. Patients may have the device removed at bedside, in an outpatient clinic, or in an operating room depending on how it was originally placed and whether tissue has grown around it.

Removal is distinct from taking out diseased or damaged native tissue - it applies only to previously implanted hardware.

Kidney

Removal from the kidney refers to taking out a device, such as a nephrostomy tube, ureteral stent positioned within the renal pelvis, or other previously placed foreign material, without cutting into or excising kidney tissue. Nephrostomy tubes are commonly placed for temporary urinary diversion in cases of obstruction, infection, or after certain reconstructive procedures, and their removal once drainage is no longer needed is a routine but clinically significant event that must be tracked separately from any therapeutic intervention on the kidney itself. The procedure may be performed percutaneously at the bedside or under fluoroscopic guidance, and typically requires little more than simple traction once the drainage catheter's retaining mechanism is released. Coders should confirm the operative or procedure note identifies a specific device or material being taken out, rather than a biopsy or tissue excision.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Device: Monitoring Device

Monitoring Device designates a device left in place to measure or track a physiologic parameter, such as an implanted pressure or activity sensor.
