Procedure Overview

Removal procedures take out a device that was previously placed in a lower joint, such as internal fixation hardware, a spacer, drainage tubing, or an external fixator frame. These procedures are performed once the device has served its purpose - for example, after a fracture has healed and screws are no longer needed - or when a device is causing pain, infection, or mechanical irritation and needs to come out. Removal can be a planned, staged part of treatment or an unplanned procedure prompted by a complication.

Because the device itself, not the joint tissue, is the focus, these procedures are typically less extensive than the original surgery that placed the device, though scar tissue and bony ingrowth around older hardware can make removal technically demanding.

The tibial surface of the left knee joint, the tibial plateau, forms the lower half of the knee's weight-bearing articulation and supports the femoral condyles through the full arc of stance and gait. Removal here addresses tibial-sided hardware alone, such as a prosthetic tibial tray, a polyethylene insert with associated fixation, or retained plates and screws from tibial plateau fracture repair, as distinguished from any procedure on the femoral component. This is performed when tibial implant loosening, subsidence, infection, or mechanical failure is identified, often as part of a staged revision knee arthroplasty. Since the tibial plateau absorbs substantial axial and shear forces during walking, surgeons take particular care during hardware extraction to protect the remaining cortical and cancellous bone for subsequent reconstruction.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
