Thoracolumbar Vertebral Disc

The thoracolumbar vertebral disc occupies the transition zone between the thoracic and lumbar spine, an area where the vertebrae shift from rib-supported to freely mobile segments and where degenerative and traumatic disc pathology commonly concentrates. A previously placed interbody cage, spacer, or disc device at this junction may be removed without replacement when fusion has been achieved, when the device has subsided or migrated, or when persistent symptoms warrant revision. Because this level, typically around T12-L1, bears substantial load during the transition from a kyphotic thoracic curve to a lordotic lumbar curve, hardware behavior and subsequent removal here can differ from purely thoracic or lumbar disc levels. The specific junctional level should be documented to distinguish this disc from its thoracic or lumbar neighbors.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
