Because the eye has many small, distinct structures, repair procedures are often highly localized, such as suturing a single layer of the cornea or fixing one extraocular muscle.

In the left eye, the choroid forms the vascular middle layer that nourishes the retina's photoreceptors and gives the fundus its characteristic reddish hue on examination. Traumatic rupture, whether from a direct blow or a penetrating wound, can shear this delicate vascular tissue and produce a suprachoroidal hemorrhage that pushes the retina and overlying structures out of their normal position. Because the choroid is so thin and richly perfused, surgical access to it is difficult and repair is typically combined with management of the scleral wound through which access is gained. The goal is to control bleeding and reappose the layer to limit ischemic damage to the retina it supplies. Records should distinguish choroidal rupture from simple hemorrhage and note whether drainage of a suprachoroidal collection was required.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
