Procedure Overview
Repair procedures restore a lower-extremity vein's wall to its normal structure when it's been torn, lacerated, or otherwise damaged, without using a different root operation that better describes the specific technique. This is the default, catch-all category for fixing a venous defect when no more specific procedure, such as bypass grafting or occlusion, applies.
A typical scenario is suturing a vein that was accidentally lacerated during another surgery, such as an orthopedic procedure near the femoral vessels, or repairing a traumatic injury from a penetrating wound. It also covers closing an iatrogenic injury caused by a misplaced catheter or needle during another venous procedure.
Because Repair is meant to bring the vessel back to normal anatomy and function, it's generally a more conservative fix than reconstructing the vein with graft material.

Hepatic Vein
The hepatic veins are the short, wide vessels draining the liver parenchyma directly into the inferior vena cava just below the diaphragm, and they are notably fragile and difficult to access surgically due to their intrahepatic course and proximity to the vena cava. Repair is coded when a hepatic vein is lacerated or injured, most often during liver trauma, hepatic resection, or transplant surgery, since these vessels can retract into the liver substance when torn, making hemorrhage control challenging. Because injury to a hepatic vein can result in massive blood loss or air embolism given its direct connection to the vena cava and right atrium, repair is often performed emergently and may require vascular clamping or specialized exposure techniques to reach the injured segment safely.
Approach: Percutaneous
Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.
