Repair is the default root operation used when an endocrine gland has been damaged - by trauma, a surgical complication, or a structural defect - and needs to be restored to its normal anatomy and function using a method not captured by any more specific root operation. Common examples include suturing a lacerated adrenal gland or closing an inadvertent capsular tear during thyroid or parathyroid surgery.
Because Repair functions as PCS's catch-all for reconstructive work on a body part, it appears whenever a surgeon fixes torn, lacerated, or otherwise structurally compromised glandular tissue without performing a more precisely defined procedure like grafting, replacement, or a named reconstructive technique. It restores the gland's physical integrity so it can continue functioning in its normal location.

Pineal Body
Repair of the pineal body is rarely performed and would apply to correcting a structural abnormality of this small midline gland, such as a defect or injury identified during surgery in the deep pineal region, when the procedure's intent is restoration rather than removal of tissue. Given the gland's location adjacent to the vein of Galen, the cerebral aqueduct, and the tectal plate, any surgical repair carries meaningful risk of disrupting cerebrospinal fluid flow or injuring nearby venous structures. This root operation would not be used for the more common pineal region procedures, which typically involve cyst or tumor excision, but rather for the uncommon circumstance where the gland's own structural integrity needs restoration. Precise documentation of the defect being repaired is essential, since procedures in this deep and delicate region are otherwise most often coded as excision, drainage, or extirpation.
Approach: Percutaneous Endoscopic
