Patients may encounter this code for anything from suture repair of a penile fracture to closure of a fistula involving the reproductive tract.

Spermatic Cords, Bilateral

When both spermatic cords require correction in the same operative episode, coding captures a single bilateral repair rather than two separate left and right procedures, most commonly for bilateral varicoceles, bilateral hydroceles of the cord, or symmetric traumatic or postsurgical defects affecting the fascia or vasculature on both sides. Each cord still runs independently from its inguinal ring to its testis, so the surgeon typically addresses them through separate scrotal or inguinal incisions even though the documentation and code reflect one bilateral body part value. Recording bilaterality accurately matters because it changes the body part character used and avoids overstating the procedure as two distinct repairs, while the underlying technique, whether suture ligation, fascial closure, or vessel repair, is otherwise identical to a unilateral cord repair.

Approach: Percutaneous

Percutaneous describes entry by needle or instrument puncture through the skin or mucous membrane to reach the site of the procedure, without cutting the tissue open or using a visualizing scope. It differs from Open in that the site itself is never exposed, and from Percutaneous Endoscopic in that no endoscope is used to see internal structures. Common examples include needle biopsies and injections.

Documentation should specify the structure repaired (testis, epididymis, scrotum, penis, vas deferens) and the nature of the defect - laceration, fistula, or dehiscence - to support the correct body part value.
