Procedure Overview

This family covers procedures that restore mouth or throat structures to their normal form and function after injury, infection, or a prior surgery, without introducing any replacement material and without simply cutting a structure out. Common examples include closing a laceration of the lip or tongue, controlling bleeding from a tonsillar bed after a tonsillectomy, mending a torn soft palate, or closing a small fistula between the mouth and a sinus cavity. The goal is always to put damaged tissue back together so it heals in its original configuration.

Patients typically need this kind of procedure after trauma such as a bite wound, a car accident, or a fall, after a surgical complication like postoperative bleeding, or after an infection has eroded tissue that then needs to be closed. Because the mouth and throat are involved in speaking, swallowing, and breathing, even a modest repair can meaningfully affect daily function, so surgeons aim to preserve as much native anatomy as possible rather than removing tissue.

Sublingual Gland, Right

The right sublingual gland lies in the floor of the mouth beneath the tongue and drains through numerous small ducts, or occasionally a single duct that joins the submandibular system, making it structurally different from the larger, single-duct parotid gland. Repair of this gland is uncommon and is generally performed for traumatic laceration of the floor of the mouth or dehiscence after excision of an adjacent lesion such as a ranula, where the surgeon reapproximates glandular tissue and overlying mucosa to prevent persistent mucous leakage into the floor of the mouth. Because the gland sits close to the lingual nerve and Wharton's duct, repair requires careful dissection to avoid injuring these adjacent structures while restoring the gland's normal position and covering mucosa.

Approach: Open

Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

A coder should look for documentation that a defect, tear, laceration, or fistula was directly closed or sutured using the patient's own tissue, without a graft that substitutes for missing structure and without an approach that removes an entire body part. The most frequent assignment error is reflexively coding suturing after tonsillectomy or another procedure as a repair when it is actually integral to the primary procedure's approach and not separately reportable; only bleeding control that becomes its own distinct procedural effort should generate a separate code. Another common mistake is confusing repair with reposition when the surgeon actually moved a structure to a new or more correct location rather than closing a defect in place.

RepositionReposition is the closest neighbor: both preserve the patient's own tissue, but reposition involves relocating a structure such as a displaced uvula or an ectopically placed salivary duct opening, while repair simply closes a defect where it already sits.
