Replacement procedures involve removing all or part of a gastrointestinal structure and putting a biological or synthetic substitute in its place, so that the substitute physically stands in for the tissue that once performed that function. In the GI system this most often appears as esophageal replacement using a segment of stomach or colon after removal of a cancerous or severely damaged esophagus, though it can also describe reconstruction of the anus or rectum with graft material following extensive resection.
These operations are reserved for situations where the native tissue cannot be repaired or reinforced and must instead be substituted outright, distinguishing Replacement from procedures that merely patch or bolster existing anatomy. Patients undergoing esophageal replacement, for example, are typically recovering from esophagectomy for cancer, caustic injury, or severe stricture disease, and the reconstructed conduit is meant to permanently take over the swallowing pathway.

Esophagus
The esophagus is the muscular conduit connecting the pharynx to the stomach, and replacement becomes necessary when disease or injury destroys enough of its length or wall that primary repair is not feasible. Indications include extensive caustic or thermal stricture, esophageal atresia in congenital cases, or resection for cancer that removes a functional segment. Because the native esophagus has no serosal layer and depends on a segmental blood supply, replacement is typically accomplished with a gastric pull-up, colonic interposition, or free jejunal graft rather than a synthetic conduit, and the substitute tissue is anastomosed to remaining native esophagus, pharynx, or stomach. Documentation should specify the conduit used and distinguish true replacement, which substitutes the biological or synthetic segment, from bypass, which routes around a diseased segment while leaving it in place.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.

Coders assign Replacement only when documentation shows both removal of the native structure and insertion of a substitute that takes over its function, not simply reinforcement of tissue left in place. The operative report should name the graft or conduit material, such as gastric pull-up or colonic interposition, and clearly describe the anatomic connection points. A frequent mistake is defaulting to Replacement whenever any graft material appears in the note, when the material may actually be reinforcing existing tissue and should be coded as Supplement instead. Another common error is failing to separately code the resection that preceded the replacement when the two are documented as distinct steps rather than one combined procedure.
