A classic example is an aortobifemoral or femoral-popliteal graft placed after removing a severely diseased, occluded, or aneurysmal segment of artery, using a synthetic conduit such as Dacron or PTFE, or in some cases a harvested vein used as a full segmental replacement. Patients typically arrive at this procedure after conservative management or angioplasty has failed, or when an aneurysm poses a rupture risk that mandates excising the weakened wall.

Peroneal Artery, Right
The right peroneal artery lies deep in the posterior calf between the tibia and fibula, often serving as the dominant or sole patent distal vessel in patients with extensive tibial artery occlusive disease, making its preservation clinically important for limb salvage. Replacement is performed when this artery itself is diseased, aneurysmal, or disrupted, most commonly as part of a distal bypass reconstruction extending toward the ankle when more direct tibial targets are unusable. Because of its deep intermuscular location, exposure requires careful dissection between the flexor muscles, and the replacement conduit, typically an autologous vein, is tunneled to reach the vessel. The relative rarity of isolated peroneal artery replacement compared to bypass grafting to it reflects its role primarily as an inflow or outflow source rather than a commonly diseased target segment requiring direct substitution.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
