Procedure Overview

Replacement procedures take out a diseased or damaged segment of an upper artery and put in biological or synthetic material, such as a graft, to physically stand in for the tissue that was removed. This differs from simply patching or reinforcing a vessel, since Replacement involves substituting material that takes over both the structure and function of the artery segment that's gone.

This approach is used when a section of artery, in the subclavian, axillary, brachial, or related upper-extremity vessels, is too diseased, aneurysmal, or damaged to be safely repaired and left in place. A synthetic graft, a segment of the patient's own vein, or donor tissue is sewn in to reconnect the artery and restore blood flow through that portion of the limb or upper body.

Replacement is a more involved undertaking than a straightforward repair, since it typically requires removing the compromised segment of vessel first before positioning and securing the substitute material in its place.

Temporal Artery, Right

On the right side, the superficial temporal artery runs just beneath the scalp anterior to the ear and is notable both as a site of biopsy in suspected giant cell arteritis and as a donor vessel in extracranial-to-intracranial bypass surgery. Replacement of a segment is uncommon and is generally performed when the artery is damaged during prior surgery, affected by trauma, or diseased in a way that precludes its use as a bypass donor, requiring reconstruction to restore reliable flow. Because the vessel is superficial and small in caliber, repair is technically delicate and often relies on fine suture technique under magnification. Documentation should distinguish a true replacement procedure from the more common temporal artery biopsy performed for diagnostic purposes in vasculitis workups.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Device: Synthetic Substitute

Synthetic Substitute designates a device made from manufactured, non-biologic material, such as mesh or prosthetic components, used to replace or augment a body part. It is distinguished from the two tissue substitute categories by its artificial composition, which carries different considerations for integration and long-term durability than biologic grafts.

Coders assign Replacement when documentation shows a segment of artery was excised or otherwise taken out and a graft, whether autologous vein, synthetic conduit, or donor tissue, was placed to take over that segment's function. The device value must reflect the specific material used, and the note should make clear that the graft is replacing removed tissue rather than being layered onto tissue that remains in place.

A frequent coding error is applying Replacement when the surgeon actually performed a Bypass, creating an alternate route around a blockage while leaving the diseased segment in place; Replacement requires that the original tissue segment be taken out, while Bypass does not. Coders also sometimes miss the distinction between Replacement and Supplement when a graft is used to reinforce rather than substitute for arterial tissue, and they should check the operative description for whether native tissue was removed before the graft was placed.

BypassBypass is the procedure most often confused with Replacement, since both can involve using graft material in the upper arteries; the key difference is that Bypass reroutes blood flow around a diseased segment that is left in place, while Replacement removes the diseased segment and substitutes material directly in its place.
