The left vertebral artery follows the same course as its right-sided counterpart, ascending through the cervical transverse foramina to help form the posterior cerebral circulation via the basilar artery, and it is frequently the dominant vessel supplying that territory in a given patient. Replacement is undertaken for dissection, aneurysmal degeneration, or occlusive disease not amenable to endovascular management, with an interposition graft used to bridge the resected segment. Given the vessel's origin from the subclavian artery and its proximity to the brachial plexus and sympathetic chain, the surgical exposure carries risk to adjacent neurologic structures beyond the cerebrovascular concerns themselves. Approach: Percutaneous Endoscopic
