Procedure Overview

Reposition procedures move a muscle, or a portion of one, to a different location, either back to its normal anatomic position after it has shifted, or to an alternate location chosen to restore function. This is common in tendon transfer surgeries where a muscle's attachment is relocated to compensate for a paralyzed or nonfunctioning muscle elsewhere, and in correcting a muscle that has slipped out of its normal position due to injury.

Because the muscle's own tissue is preserved and simply relocated rather than removed or replaced, these procedures aim to restore or redirect function using tissue the patient already has. Recovery typically requires a period of protected immobilization followed by therapy to retrain the muscle in its new role or position.

Abdomen Muscle, Left

The left abdomen muscle group, including the rectus abdominis and the external and internal oblique muscles, contributes to the structural integrity of the abdominal wall and is frequently involved in reconstructive procedures on this side. A muscle in this group is repositioned when it is mobilized during component separation for complex hernia repair or when the rectus abdominis is rotated on its pedicle to reconstruct a defect elsewhere in the body, as in some flap-based breast or pelvic reconstructions. Because the abdominal wall muscles have overlapping fascial layers and a segmental nerve and blood supply, precise identification of the specific muscle relocated and its resulting position is necessary to differentiate reposition from a straightforward repair or resection in the same region.

Approach: Percutaneous Endoscopic

Percutaneous Endoscopic combines a puncture through skin or mucous membrane with insertion of an endoscope to visualize the procedure internally, as in laparoscopic cholecystectomy. It is distinguished from plain Percutaneous by the presence of scope-guided visualization, and from Via Natural or Artificial Opening Endoscopic by its route being a new puncture rather than an existing orifice or stoma.

Reposition applies when the documentation describes moving a muscle, or detaching and reattaching it, to a new anatomic location, whether that location is the muscle's own normal position or a different site chosen for functional benefit, as in a tendon transfer. The operative note should specify the muscle involved and its new attachment site clearly enough to select the correct body part value.

A frequent mistake is coding a muscle transfer procedure as Transfer, which does not exist as a root operation in the Muscles body system in the way it does for skin or subcutaneous tissue, when Reposition is the applicable term for relocating muscle. Coders should also distinguish whether the muscle is being moved with its blood supply intact and left attached at its origin, which still qualifies as Reposition, from a free graft situation.

Commonly Confused With

TransferReposition is often confused with Transfer as used in other body systems, but within Muscles a relocation of tissue, whether pedicled or otherwise, that changes the muscle's location is captured under Reposition rather than a separate Transfer root operation.
ReplacementIt is also confused with Replacement, since both can involve significant surgical movement of tissue, but Reposition keeps and relocates the patient's own existing muscle rather than substituting new material for missing tissue.
