Procedure Overview
Reposition procedures move a respiratory structure to its correct anatomic location without cutting any of it away or adding replacement material. The classic example in this body system is diaphragmatic plication, where a surgeon folds and sutures an abnormally high, flaccid diaphragm, often caused by phrenic nerve injury, back down into its normal position to improve breathing mechanics. Correcting a displaced or twisted lung lobe (torsion) after surgery also belongs here.
These procedures are chosen when the underlying tissue is intact and functional but sitting in the wrong place, as opposed to situations where tissue must be removed or reconstructed. The goal is purely mechanical realignment so the structure can do its normal job again.

Diaphragm
The diaphragm is the dome-shaped musculotendinous partition separating the thoracic and abdominal cavities, essential to normal respiratory mechanics through its rhythmic contraction and relaxation. Reposition of the diaphragm is coded when this structure is surgically moved to a new location, commonly performed to correct diaphragmatic eventration, where a weakened or thinned portion has bulged abnormally into the chest, or to relocate the diaphragm following plication surgery for paralysis or paradoxical movement caused by phrenic nerve injury. Because the diaphragm's position directly affects lung expansion and abdominal organ containment, repositioning procedures often involve suturing redundant or displaced tissue to restore a more normal contour, and documentation should distinguish this relocation from repair procedures addressing structural defects such as hernias.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
Coding & Documentation
Coders need documentation that the surgeon relocated an existing body part to a normal or otherwise suitable position, distinct from simply repairing a hole or defect in place. For diaphragm cases, the note should describe plication or folding to reduce an eventration, not just suturing a tear. A common error is coding diaphragm plication as Repair because both involve suturing, when the defining action is repositioning redundant tissue rather than closing a defect.
