Procedure Overview
Reposition describes surgically moving a body part to its normal location, or to another suitable location, without cutting it out or replacing it. The clearest example in the male reproductive system is orchiopexy, where an undescended testis is moved from the abdomen or inguinal canal down into the scrotum and secured there. Reposition may also apply to correcting testicular torsion, where the testis has twisted on its spermatic cord and needs to be untwisted and fixed in place to prevent it from happening again.
These procedures are performed to preserve fertility and hormone-producing tissue, reduce cancer risk associated with undescended testes, and relieve the pain and tissue damage caused by torsion. Timing matters clinically - torsion in particular is often a same-day emergency because delay can mean loss of the testis.

Testes, Bilateral
Bilateral repositioning of the testes is performed to correct undescended or maldescended testicles, most often in an orchiopexy for cryptorchidism identified in infancy or childhood, or to address testicular torsion where both sides are at risk and fixation prevents recurrence. Each testis is mobilized from an inguinal or abdominal location, its blood supply and vas deferens preserved, and the gland is moved into the scrotal sac where it is anchored, typically to the dartos fascia, to hold it in normal anatomic position. Because the procedure is bilateral, documentation should reflect that both testes were relocated in the same operative episode, distinguishing it from a unilateral repair. Timing matters clinically, since delayed correction of undescended testes carries implications for fertility and malignancy risk, factors that often drive the decision to operate.
Approach: Open
Open approach means the surgeon cuts through skin, mucous membrane, or other tissue layers to physically expose the target site to view before performing the procedure. It gives direct visualization and hands-on access, distinguishing it from Percutaneous approaches where instruments pass through a small puncture without exposing the site. Open is typical for procedures needing wide access, such as most laparotomies.
Coding & Documentation
The operative note needs to show that tissue was mobilized and relocated, with fixation to hold it in the new position, for Reposition to apply. A common coding pitfall is applying Reposition to a torsion case that ultimately required removal of a nonviable testis - that outcome is Resection or Excision, not Reposition, and should be coded from the findings, not the original surgical plan. When both testes are explored in bilateral torsion, coders should verify whether both were fixed, only one, or one was fixed while the other was removed.
